- Enquire sensitive information about patients at the nurses’ office.
- Take over ward from bed to bed verifying the state of all patients; especially the seriously ill.
- Establish rapport with patients during taking over and ask about general health.
- Conduct inspection of ward with the outgoing staff.
- Ensure resources needed for work are available and adequate and take over controlled medication.
- Counter-sign written ward report.
- Note important issues and document.
- Congratulate out-going staff.
